Designed for foreigners who intend to stay in the Czech Republic for a period longer than 90 days up to 365 days for health purposes, including long-term rehabilitation therapies, spa services, surgery, and other health-related reasons. Requires confirmation of medical procedure, hospitalization, invitation, or medical report. This visa does not permit employment.

Visa validity period
The period during which the visa remains valid and can be used for entry into the country. The validity period or expiration date is indicated directly on the visa itself.
A long-term visa is granted for activities exceeding 90 days that require the applicant's continuous presence in the Czech Republic. It is issued for a maximum duration of one year.

Duration of stay
The maximum duration for which one is permitted to stay in the country continuously from the date of entry.
The stay period for a specific long-term visa is determined by the consulate, depending on the individual situation of the applicant and documentary evidence of the purpose of the visit. Long-term visas are granted to third-country nationals intending to stay in the Czech Republic for more than three months for various purposes. These visas are issued for a maximum period of 1 year.

Extension of stay
The extension of the maximum duration of stay in the country initially permitted by the visa, subject to approval by the immigration authorities. This is typically processed within the country without the need for departure.
A long-term visa issued for less than 1 year can be extended if the purpose of stay remains. A long-term visa including extensions can be valid in total for a maximum of 1 year. If the applicant wants to stay in the Czech Republic for a period longer than one year, he must apply for a long-term residence permit.

Number of entries
A parameter specifying the number of times the visa holder can enter the country during its validity period. Visas can be single-entry, multiple-entry, or with a specified number of entries.
A long-term visa grants the possibility to leave and re-enter the Czech Republic repeatedly. Holders have a right of free movement in the Schengen Area for a maximum of 90 days within any 180 days for non-gainful purposes (in this respect, it fulfills the function of a short-tem Schengen visa).

Processing time
The period during which the immigration authorities review the application and make a decision regarding the issuance of the visa. Only business days are counted, excluding weekends and public holidays.
Decisions on a long-term visa are made within 90 days. The legal processing time can be extended to 120 days in individual cases requiring more examination. The processing time is suspended if the proceedings are put on hold if there is a legal ground for suspension.

Consular fee
A fee levied for the processing and evaluation of an application. This charge is non-refundable under any circumstances. In certain instances, the payment of supplementary fees may be required.

Passport
Issued less than 10 years ago, with a validity period exceeding the duration of the visa by at least three months and with at least two blank pages.
Photograph
Taken in the last 6 months, the photograph must be 45×35 mm, rectangular with straight or rounded corners, and thickness between 0.13–0.27 mm. It must show the applicant’s head and upper shoulders in full-face view, with at least 2 mm of space above the head. The face height (eyes to chin) must be at least 13 mm. The applicant must face the camera directly, with a neutral expression, closed lips, and uncovered open eyes. Retouching or alterations are not permitted, and the image must be free of reflections obscuring facial features. Photographs may be in black-and-white or colour.

Internationally recognized payment card
Applicants must have a valid internationally recognized payment card accepted for use in the Czech Republic.
Proof of financial means
Bank statement or any other proof of income, demonstrating sufficient funds to support oneself for the entire stay and for return expenses. Applicants must prove they have 46,950 CZK (equivalent to funds in the amount of 15 times the subsistence minimum - 3,130 CZK per month) and twice the subsistence minimum for every whole month (except for the first month) of their expected stay in the country.
Sponsorship letter
In cases of sponsorship by a host, public authority or legal entity, an invitation letter outlining financial commitments or other evidence that all expenses related to the stay will be covered must be provided.
Proof of accommodation
Confirmed lease or sublease agreement, confirmation of ownership of the apartment or house. Proof of accommodation must be valid for the entire duration of the visa procedure (or at least three months from the date of the application submission)
Parental consent
For applicants under 18 years of age, a document is required in cases where the child will be staying in the Czech Republic without the presence of their parent or legal guardian. This document must include the consent of the parent, legal representative, or guardian, confirming authorization for the child's stay and ensuring the appropriate arrangements for the child's care and supervision in the country.
Criminal record extract
Confirming no criminal record in the applicant’s home or residence country, where they resided for a total of 6 months during the 3 years preceding the visa application.
Medical certificate
In some cases, citizens or residents of some countries may be required to provide health certificates confirming that they have no easily transmitted diseases such as tuberculosis or other serious diseases that pose a risk and danger to large numbers of people.
*Foreign official documents must be apostilled and legalized in accordance with the regulations of the country whose consular or diplomatic mission they are being submitted to. Additionally, they must be accompanied by a certified translation into the official language of that country or into English, unless otherwise specified by the mission's requirements.
